PR Summary

This PR adds some additional useful info and questions to the issue template.

We get lots of issues with a mix of information that would be beneficial to have asked for as part of info we ask when issues are raised.

I think this should have CL-Tools as the CL Label but wasnt 100% sure so haven't added this
